Case Studies
AIM engages in reducing LOS and excess days through interdisciplinary rounds, involvement and care team engagement with nursing, ancillary support, and consultants resulting in early detection and resolution of discharge barrier’s. AIM collaborates with specialists on care pathway improvements and refining care processes to reduce unnecessary utilization and increase throughput.
AIM’s patient-focused service and targeted engagement of care coordination from admission through discharge, includes their Nurse Coordinator Program and Post-Discharge Call Program. These collaborations and supportive roles have provided exceptional opportunities to reinforce physician communication, service recovery, boost patient satisfaction scores by keeping families engaged in their loved one’s care and assists in the identification and resolution of barriers to discharge reducing unnecessary excess days.
AIM engages in a teamwork approach to develop and implement successful clinical documentation improvement strategies (in understanding and in practice) to advance the quality, accuracy, and completeness of physician’s clinical documentation for capturing SOI, and correctly measuring CMI and reflecting GMLOS improvements accurately.
